ANNUAL CELEBRITY GOLF BENEFIT REGISTRATION
 
If you are registering an individual golfer simply leave the team name field blank and place the golfer's name in the player 1 field.  If you are registering a team you must provide a team name and all 4 player names.  If you have not yet confirmed all of your team players you must still include 4 player names.  You may make substitutions on event day.
 

Registration Fee is $200.00 per player or $800.00 per team.
